1. Open the form that pertains to you 

   • Work Permit Under 16

   • Work Permit ages 16 -17
     
     

2. Fill out Section I.
   
   

3. Take the form to your employer/potential employer. They need to fill out Section II.
   
   

4. E-mail the form to your school office.

5. The school will complete Section III and email the form back to your student e-mail.

*Work permits can also be dropped off for signature at Oxford High School (745 N. Oxford Rd.) during the summer break between 9am and 2pm.
