The Mask Exclusion Form is now available to be printed, completed, and returned. It can be found on our website with our Medical Plan forms.
Additionally, we will be honoring any medical Mask Exclusion Forms we received last year for the time being, but will require up-to-date documentation prior to second semester. All forms can be sent to [email protected].
